Coordinating Client Care and When to Refer Out

Course Description: This course is designed to enhance the ability of peer support specialists and health care professionals to recognize critical patient risk factors that may require specialized care or intervention. The presentation will cover key indicators of risk across various health domains, including physical, mental, and behavioral health, as well as red flags that signal the need for referral to specialists or other providers. With a special focus on working with teen populations, the presentation explores unique risks faced by adolescents, including mental health challenges, substance use, and behavioral concerns, as well as effective strategies for engaging with teens and their families. Participants will learn evidence-based strategies for assessing risk, effective communication techniques for discussing referrals with patients, and how to build collaborative relationships within the broader healthcare network to make successful and strong referrals.
